Applicants must show evidence of support for the formation of a Friends of the Library. In the 'Evidence of Support' section below, please provide names, addresses, and phone numbers of the organizing team who will be responsible for the grant. Communities of 5,000 or less may submit only three names. Communities of more than 5,000 must submit five names.

If you can enter a dollar amount in the 'Amount of local financial support for organizing efforts' field, we are a candidate for additional funds from the Charles and Lynn Schusterman Family Foundation Challenge Grant to help support the seed grant.

Applicants must present the above information for consideration by FOLIO by the first day of the month in September, December, March, or June.

Provide membership list of the Friends Group within a year of the grant application.

Applicants will receive notification of the decision of the FOLIO Board following the next board meeting after the receipt of the application
